Background/ Aims
We sought an innovative and single source of information through an electronic platform to improve communication with consumers that is standardised, accurate, transparent regarding TIA clinic appointments, post TIA clinic information, patient education and patient survey of service.
Inconsistencies in the format and method that consumers were contacted by the TIA Nurse Consultants and received clinic information.
Upon review of consumer survey, consumers had difficulty finding locations at the Royal Adelaide Hospital and indicated that they were seeking more information on their medical condition and medications both Stroke and non stroke.
Method
Personify was selected to trial as it is a standardised contemporary electronic platform that was both innovative for TIA clinic and met SA Health recommendations. Personify was able to embrace technology to enable excellence. Pathways can be modified to customise it for use by different services.
Partnered with consumers to discuss role of platform, requirements and to review the pathway.
Results
Average of 92% uptake in registration over initial 6 month period.
Patient surveys indicate 90% of those accessing the information found it useful
However uptake of provided information by patients was only 30%
96% of patients gave 4/5 star reviews for care in TIA Clinic
Conclusion
Use of electronic care pathway provided improvements in two way communication with TIA Rapid Assessment Clinic patient for information and secondary prevention advice.
However reflection on the implementation and uptake brought up need to audit percentage of consumers declining to use Personify and what the barriers are.
