The Stroke Action Plan for Europe (SAP-E) is the European Stroke Organisation/Stroke Alliance for Europe framework to reduce stroke burden by 2030 through 13 key performance indicators (KPIs) spanning prevention, acute care, rehabilitation and life after stroke. Stroke Service Tracker 2023 data from 47 European countries documented major inequities in service delivery; only 20 countries (43.5%) had a national stroke plan and 20 (43.5%) had a quality programme, both of which were associated with higher KPI attainment. Against this background, telemedicine and virtual care should be viewed as system enablers rather than isolated technologies. In the hyperacute phase, hub-and-spoke telestroke networks, prehospital digital triage, remote imaging review and virtual stroke expertise can expand access to stroke unit care, intravenous thrombolysis and mechanical thrombectomy, with the potential to improve time-sensitive workflows and reduce geographical inequity. Beyond the acute setting, virtual medicine can support SAP-E goals in secondary prevention by enabling structured 3–6-month follow-up, remote blood pressure and rhythm monitoring, medication optimisation, adherence support, lifestyle intervention and rapid specialist reassessment after discharge. Tele-rehabilitation, electronic patient-reported outcomes and shared digital care pathways may further strengthen continuity across rehabilitation and life after stroke. Integrating telemedicine within national stroke plans, quality programmes and interoperable audit systems could therefore accelerate progress toward SAP-E KPIs across the full care continuum. This strategy is especially relevant for regions with limited specialist availability, where virtual models may improve equity, efficiency and long-term outcomes.
